What sildenafil actually does
During arousal the body releases nitric oxide in penile tissue. That raises cGMP, which relaxes muscle so blood can fill the penis. The enzyme PDE5 breaks cGMP down. Sildenafil blocks PDE5, so the signal lasts longer — but only if arousal is already there. Without stimulation the jelly will not force an erection, and it does not protect against sexually transmitted infections.
